Mercer Health Private Duty
Mercer Health Home Care Private
Duty provides Duty and Respite to the client/caregiver in
their place of residence. Service
are given to clients under an approved plan of care for
the purpose of promoting, restoring and maintaining or
minimizing the effects of illness or disability. Care will
be given on an agreed contract on an hourly basis, based
upon client or family request.
Geographical Area We Serve
Services
are provided to clients who reside in Mercer County and
fringe area's of Van Wert, Auglaize, Shelby and
Darke counties.

Referrals
Request for services may be made by the physician, client,
family member or health facility. All services provided
are under a physician's plan of care.

Coordination of Supervision of Services
The Mercer Health Home Care Private Duty program will coordinate
services provided to enhance the comprehensive care of
the client. Referrals to the appropriate community resources
will be made. We are required to provide supervision of
the Personal Care Aide, therefore, the supervisory visit
will be performed by an Registered Nurse during a home
visit usually every 2 months of as needed.

Disclaimer
Every attempt will be made to provide services for the times
agreed upon. In the event of unforeseen circumstances coverage
may not be available and the scheduled visit may be cancelled.

Concerns
We want to work with you and your family to provide quality care and ensure
a positive experience. In the event that there is a problem, please feel free
to discuss it with a staff member, or our Home Care Director. She will work
with you to resolve your concern. There is another option available to you
as well. The Ohio Department of Health has set up a toll-free hotline phone
number for
you to call in the event of a complaint about our agency or for any concerns
related to the implementation of Advance Directives. you may contact them
