At Mercer Health Home Care we know that the transition from the hospital to your home can be hard. That's why at Mercer Health Home Care we make it as easy as possible by assisting you in setting up any home services you might require. Whether it be delivering and setting up an electric hospital bed or bathroom accessories Mercer Health is there.

Discharge Planning

When you are ready for discharge from our agency we will assess your total medical, psychosocial, and financial process to be able to manage your illness in the community.  We will assist you in transitioning from the help of home care to working with community resources.  No two people are exactly alike and we individualize your planning with you starting the very first day and continuing until you are ready.

*Mercer Health Home Care does not physically transport the patient from the hospital to their home, we only deliver medical home accessories*
