• Unless asked otherwise, patients will always be addressed
as "Mr.", "Mrs.", "Miss",
or "Ms."
• Mercer Health Home Care staff members always keep names, medical conditions, treatments,
and
family
information private.
• None of our patients information is not released unless appropriate authorization
and
consent
has
been
obtained.
At Mercer Health Home Care we create a personalized chart for each of our patients — containing
all physician's orders, notes and pertinent medical information. Staff members
use
these charts
to record patient treatments and medical conditions. A patient can request to
see their information at any time through an appropriate request. Only individuals
with
legal
rights, or those with written permission by the patient, can view a patient's
chart.
Problem Solving Procedure
Mercer Health Home Care protects our patients' rights. However, any concern or
problem
the you might have can
be expressed by contacting the clinical
supervisor. Additionally,
Mercer Health Home Care's Main Office can be contacted to report
any problems or
concerns that you might have with our service.
