About Mercer Health Home Care

Mercer Health Home Care provides skilled services to the client in their place of residence. Home Care is given to clients under a physician approved plan of care for the purpose of promoting, maintaining and restoring health or minimizing the effects of illness or disability. Visits are made on an intermittent defined as a medically predictable reoccurring need for skilled services. Home Care is a department of mercer health.

Our Hours of Service
Mercer Health Home Care provides services 24 hours a day, 7 days a week. Our office hours are Monday through Sunday, 8:00 a.m. - 4:30 p.m.

After hours, a registered nurse is on-call for any problems that may occur. Please call the hospital at 419-678-2341. Leave your name and phone number with the operator and she will contact the nurse. The Home Care Nurse will call you as soon as possible.

If an emergency situation occurs, do not hesitate to call 9-1-1 or your local emergency rescue squad.

Geographical Area We Serve

Services are provided to clients who reside in Mercer County and fringe areas of Van Wert, Auglaize, Shelby, and Darke counties.

Referrals
Request for services might be made by a physician, client, family member or health care facility. All services provided are under the physician's plan
of care.

Coordination & Supervision of Services
The Home Nursing Care Program will coordinate
services that will enhance the comprehensive care
of the client. Referrals to the appropriate community
resources will be made. The supervisory visit is done
by an RN on a home visit usually every 2 months or
as needed.

Services We Offer

Skilled Nursing is provided by a Registered Nurse
in accordance with the plan of treatment. The Nursing
process is utilized to assess your health problems,
develop a plan of care, implement and evaluate the effectiveness of this care. The Registered Nurse will coordinate services needed and inform the physician and other personnel of changes in your condition or needs.

Physical Therapy is provided by a Physical Therapist or a Physical Therapist Assistant. Physical Therapy is concerned with improving or restoring mobility of all limbs which have been impaired. The type of therapy administered is designed specifically to your needs. The goal is to help you function as independently as possible.

Occupational Therapy is provided by an Occupational Therapist in accordance with the plan of treatment. Occupational therapy is concerned with improving or restoring functional abilities to the client who has been impaired due to illness or injury.

Speech Therapy is provided by Speech Therapist who is concerned with improving or restoring language and speech abilities which have been impaired due to illness or injury.

(The above services are considered Skilled Services. Home Care Aide services cannot be provided unless one of the above services is needed.)

Home Nursing Care Aide services are provided by an experienced Home Care Aide in accordance with the plan of treatment. Services include personal care, bathing, grooming, ambulation, exercises, and household task essential to health care at home. These services are supervised by the registered nurse.
