Transferring

At Mercer Health Home Care we know that the transition from the hospital to your home can be hard. Mercer Health Home Care makes it as easy as possible by assisting you in setting up any home services you might require. Whether it be delivering and setting up an electric hospital bed or bathroom accessories Mercer Health is there.

We do not physically transport the patient from the hospital to their home.

Discharge Planning

When you are ready for discharge from our agency we will assess your total medical, psychosocial, and financial status so you can manage your illness in the community.  We will assist you in transitioning from the help of home care to working with community resources.  Since no two people are exactly alike, we individualize your planning with you. We will work with you on the very first day and continue until you are ready to be on your own.
