Geographical Area We Serve

Mercer Health Home Care Services are provided to clients who reside in Mercer County and fringe areas of Van Wert, Auglaize, Shelby, and Darke counties.

Referrals
Request for services might be made by a physician, client, family member or health care facility. All services provided are under the physician's plan of care.

Coordination & Supervision of Services
Mercer Health Home Care will coordinate services that will enhance the comprehensive care of the client. Referrals to the appropriate community resources will be made.
