About Mercer Health Home Care

Mercer Health Home Care provides skilled services to the patients in their place of residence. Home nursing care is given under a physician approved plan of care for the purpose of promoting, maintaining and restoring health or minimizing the effects of illness or disability.

Visits are made based upon a customized plan of care defined by a medically predictable re-occurring need for skilled services.

Our Hours of Service
Mercer Health Home Care provides services 24 hours a day, 7 days a week. Our office hours are Monday through Friday, 8:00 a.m. - 4:30 p.m. Our office number is 419-584-0143.

After hours, a registered nurse is on-call for any problems that may occur. Please call the hospital at 419-678-2341. Leave your name and phone number with the operator and she will contact the nurse. The Home Care Nurse will call you as soon as possible.

If an emergency situation occurs, do not hesitate to call 9-1-1 or your local emergency rescue squad.
