Mercer Health Home Care provides skilled services to the patients in their place of residence. Home nursing care is given under a physician approved plan of care for the purpose of promoting, maintaining and restoring health or minimizing the effects of illness or disability.

Visits are made based upon a customized plan of care defined by a medically predictable re-occurring need for skilled services.

Our Staff of Professionals includes:

  • Registered Nurses that perform patient assessments to determine home care needs.
  • Certified Ostomy/Wound Nurse assists patients with all care related to Ostomy and Wounds.
  • Home Care Aides to assist with bathing, dressing, and other daily routines.
  • Physical, Occupational, and Speech Therapists to help the patient in their recovery after surgery or other health problems.
  • Registered Dieticians to help with nutrition counseling.
  • Social Service Personnel to help with placement/family issues or financial assistance.
  • Licensed Respiratory Therapists to assist the patient in their respiratory needs.
